Take five minutes

Use these prompts to organise your thoughts. There is no score and no pass or fail; this is a planning check-in, not a medical examination.

  • Energy: what felt easy or unusually tiring this week?
  • Rest: have you had time and a comfortable setting for sleep?
  • Food and fluids: what would make regular meals and drinks easier?
  • Movement: what activity did you enjoy, and who could join you?
  • Mood and connection: who can you talk to, and what needs support?

Make the next step small

Pick one action you can fit around your life: a short walk with someone, preparing lunch, or leaving time to unwind. Enjoyable activity and social connection can support wellbeing. They sit alongside professional care when that is needed.

Plan an actual health appointment

Ask your GP which checks suit your age, history and current concerns. The conversation may include blood pressure, cervical or breast screening, and other checks relevant to you. A shopping checklist cannot tell you which tests you need. Bring a note of symptoms, medicines and questions.
